TO REQUEST AN APPOINTMENT:

Your Full Name
Preferred Appt Date/Time
Reason for Visiting
Preferred Services

Once your request is received, we will send a reply via email.

Completion of waivers/release forms and full payment are required to retain your scheduled appointment. Reservations for appointment are not confirmed/complete until these steps have been finalized.

Although we love and respect all beings, we have the right to refuse services to anyone, for any reason or without reason.

MISSED APPOINTMENTS, RESCHEDULING & REFUNDS

If you are unable to attend your appointment for any reason, please notify us as soon as possible.

Your payment is a retainer for the time you are requesting and is non-refundable.

Missed appointment with prior notice - you may request a new appointment, no additional payment required, no refunds.

Missed appointment without prior notice - forfeiture of full payment, new payment for rescheduling required.

2+ missed appointments without prior notice will result in termination of client/provider relationship, future scheduling requests will be denied.
